Chronic Disease Management/Diabetes Management
What is a Chronic Disease?
Chronic Disease is a long-lasting condition that can be controlled but not cured. Patients suffering from chronic disease illnesses (present for six months or more) are managed closely at Health on Kensington to achieve the best health outcomes.
GP Management Plans and Team Care Arrangements can be created for these patients. These documents are created to target a patient's specific needs relating to current physical, mental health and support services.Several providers may be involved in actioning these plans, such as Doctors, Nurses, Specialists, Allied Health Providers (Physiotherapists, Podiatrists, Pharmacists, Chiropractors, Community Nurses and other local agencies).
Patients who have a GP Management Plan and Team Care Arrangement can also access five visits over 12 months with an allied health provider. This extra professional support may assist with acute situations such as joint issues or provide long term surveillance of potential complications such as treatment by a podiatrist.
These plans assist the patients in understanding their care, managing referrals, and timing of visits and tests. This is a chance for patients to be reminded of services or new treatments available to make sure they are receiving the best possible care.
Diabetes and Asthma can also be closely managed with yearly intensive reviews. We review blood results and other investigations with the patient, focusing on achieving the best possible health without disease complications.

Over 75 year-old Heath Assessment
Once a patient reaches 75 years of age (55 years of age for Aboriginal & Torres Strait Islander patients), they are entitled to an in-depth Health Assessment, provided by our practice nurse and GP of your choice.
Patients can take advantage of this assessment yearly, but please discuss with either a Nurse or doctor if the need arises for assessment sooner.This assessment is bulk billed.

- Blood Testing – INR/Blood Sugar Levels
- Immunisations – Adult and Child

Some Nursing services may incur a Nurse fee or a supply charge.
Q Fever Vaccinations
Q Fever is an infection caused by Coxiella burnetii, a type of bacterium found worldwide, except New Zealand. The infection is almost always related to direct or indirect contact with animals such as cattle, sheep or goats, although a wide range of animals including cats, dogs and kangaroos may carry the infection.
Q Fever vaccination is a two step process. Firstly, a skin test and blood sample on the first visit, and seven days later, patients return. On the second visit, the vaccination is administered depending on the patients results from the prior test.
Dr Lorenzo Ponte is qualified to administer the Q Fever vaccine. Please make it known to reception you are requiring the Q Fever vaccine at time of booking so the appropriate booking can be made.
Childhood Immunisations
Childhood immunisations are booked with the Practice Nurse and the doctor. Our practice nurse is wonderful with immunising children, and will make the experience as stress-free as possible for both the parent and child.

Mental Health
All of our doctors see patients for Mental Health related appointments. If your appointment is solely to discuss your Mental Health, generally allow 30 minutes for these appointments as often more time is required.
Skin Cancer Checks and Treatment
We offer Skin Screening appointments conducted by Dr Lorenzo Ponte to check for possible Skin Cancers.
Medical Examinations
Pre-Employment Medicals
Health on Kensington is qualified to complete a wide range of Pre-Employment Medicals.
The majority of these bookings require 30 minutes with our Practice Nurse followed by 15 minutes with the GP. However, the required length may depend on the detail being requested by the employer. Testing may include Urine Drug Screening (offsite), Radiology Scans and Blood Testing. *Please note: Urine Drug Screening is NOT instant. Pathology is sent offsite and results take up to 7 days.
If you have not yet had a medical completed by Health on Kensington, we require a copy of all paperwork to determine appointment length and complexity.
All completed documentation will be posted registered post (or emailed upon written request) by Health on Kensington directly to your Employer/Company. No paperwork will be given to the employee / patient at the completion of the consult.
Accounts need to be paid within 14 days.

Driving Medicals - Heavy Vehicle/Road Train/Dangerous Goods/Passenger Vehicle Accreditation
Transport SA has a number of different forms that need to be completed by the Doctor i.e. Light Vehicle, Heavy Vehicle, Road Train, Commercial Vehicle and Dangerous Goods Medical. Please have these forms with you when you’re making the appointment. The consult may take 30-60 minutes.
